Dr. Rubiales is a provider with the following insurance companies: Providence, Cigna, Regence Blue Cross Blue Shield, ODS and Moda Health Plan. However, eligibility and coverage may vary for individual plans. The office also bills for Personal Injury claims related to motor vehicle accidents, Worker’s Compensation, and for some other plans as an out-of-network provider.
